  3. PERSONAL APPEARANCE CONTRACT 
  4.  
  5. _______________________________, referred to as TALENT, and
  6. ________________________________, referred to as SPONSOR,  agree: 
  7.  
  8. TALENT shall appear on ______________________________ from
  9. ________________ to ________________ at a
  10. ________________________________________ for  SPONSOR.  
  11.  
  12. TALENT shall provide the following equipment:  
  13.

  15. At least two working days prior to the appearance the SPONSOR
  16. shall insure that adequate support facilities are provided for the
  17. equipment as follows: 
  18. ______________________________________________ 
  19. SPONSOR shall complete and return to TALENT the attached "Remote
  20. Checklist form" two business days prior to the scheduled event. 
  21.  
  22. SPONSOR is responsible to provide the agreed upon support 
  23. facilities or personnel. In the event that the support facilities 
  24. agreed upon are not provided, the TALENT may cancel or abridge 
  25. the appearance; however, the full agreed upon fee shall be paid. 
  26.  
  27. SPONSOR shall provide the following equipment and support:

  31. SPONSOR shall at least two working days prior to the appearance
  32. verify that all equipment and personnel support stated are 
  33. prepared and shall submit a "Remote Checklist form" two business
  34. days prior to the scheduled event. 
  35. SPONSOR is responsible to provide the agreed upon equipment. In
  36. the event that the agreed upon equipment and support are not
  37. provided, the TALENT may cancel or abridge the appearance;
  38. however, the full agreed upon fee shall be paid.

  40. The SPONSOR shall pay to talent a total fee of $  ______________

  42. dollars) for the appearance. This fee shall be due, in full,
  43. unless the event is canceled a minimum of _________________ hours
  44. prior to its scheduled start. The appearance may be extended
  45. provided that the TALENT agrees. An additional fee of

  47. no/100 dollars) per half-hour shall be due. The TALENT may
  48. condition the extension of the appearance on prepayment.

  51. Food and beverage shall be complimentary to TALENT during the

  55. TALENT in connection with the appearance.

  59. This is the entire agreement between the parties, and there are no
  60. other agreements. This contract may only be modified in writing.
